The 2021 Community Health Needs Assessment

Your Input Matters!

Please participate in the 2021 Community Health Needs Assessment by completing a 5 minute survey. Every survey completed will be utilized and together they create a "roadmap" of input that many organizations -governmental, public, and private- can all use and follow to address health needs in our community. This is how you can use your voice to help us know what is important to you and your family. We want to hear from you!

Thank you for taking the time to assist your community in a way that truly matters. Access to HealthCare is very important and this is a way to examine the health issues of the communities within our county. It will assist in identifying health priorities that will then be the focus for community health improvement during the next three year cycle. 

This CHNA cycle, we are not only partnering with Health ENC to conduct our assessment; We are also taking a "tri-county approach" and collaborating with: Carolina East Health System, Craven County Health Department, and Jones County Health Department to complete the CHNA process. This collaboration will allow us to have a "True Regional" picture of what our Health Needs are in this area of our state.

We are currently on Phase 2 of this CHNA's 8-Phase process: "Collection of Primary Data." We are looking for assistance in gathering Primary and Secondary data as well. Please let us know if you are interested in assisting us to collect this priceless information from our community.
